In order to close your account and stop taking responsibility for the consumption, you will need to:

  • Complete the Business Move Out Form (below).
  • Monitor your email address for a Confirmation email from Eglinton Village Energy. This will include important information about the request.

To assist us to provide you with services, we need to collect personal information about you. We will treat your information in accordance with our privacy policy, a copy of which is available upon request. If you have Life Support equipment at your home or premise, we want to ensure that you will be safely connected and understand how to register, so please call us on 08 9416 2026. Note: All fields marked with an asterisk (*) are mandatory. Please fill the information in CAPITAL letters.

Note, if you select email copy, we will send you correspondence and invoices by email and you will not be provided with a paper copy of correspondence or your EVE invoices.

This is the date you want your Account to be closed.


Part 5: Account Holder Signature Panel

You understand that your Customer Account will be closed using the information in this form, and that this request relates to the cessation of the supply of electricity at the address listed in Part 2. The Account will be closed on the Move Out Date populated in Part 4, and the disconnection will occur at any time between 6:00am and 8:00pm on the Move Out Date. Your Final Bill will be sent the following month to the forwarding address in accordance with the Correspondence Preference nominated in Part 3. By signing this form, you are confirming that all the information in Parts 1 to 4 above is accurate and true, and you agree to the terms provided in this form, acknowledging that there are also terms of your supply contract with us that will survive termination, in accordance with its terms.
