To assist us to provide you with services, we need to collect personal information about you. We will treat your information in accordance with our privacy policy, a copy of which is available upon request. In this section, we will ask you for information that will enable us to close your Customer Account. We will need updated details, particularly your contact number, email address or mailing address, so your Final Bill will be issued to the right place, and we can contact you if required.

You understand that your Customer Account will be closed using the information in this form, and that this request relates to the cessation of the supply of electricity at the address listed in Part 2. The Account will be closed on the Move Out Date populated in Part 4, and the disconnection will occur anytime between 6:00am and 8:00pm on the Move Out Date. Your Final Bill will be sent the following month to the forwarding address in accordance with the Correspondence Preference nominated in Part 3. By signing this form, you are confirming that all the information in Parts 1 to 4 above is accurate and true, and you agree to the terms provided in this form, acknowledging that there are also terms of your supply contract with us that will survive termination, in accordance with its terms.
